Intraoperative aneurysm rupture may have catastrophic consequences. When the parent artery is forced to permanently block, not only does the mortality rate increase significantly, but serious neurological impairments are often left behind. This article retrospectively analyzed 276 operations of 317 aneurysms and 41 aneurysms ruptured in the perioperative period or during surgery. Five ruptures occurred before the craniotomy, and three of them occurred during the induction period of anesthesia. These cases were not analyzed in this article. Of the remaining 36 cases, anterior communicating aneurysms accounted for 37% and basilar aneurysms accounted for 17%, both far exceeding their proportion in the total number of 317 cases in this group. The rate of rupture of internal carotid aneurysms is essentially equal to their ratio in the total number of cases. Aneurysm rupture in the brain accounts for 12% of the total. The author divides the prognosis of aneurysm rupture into two types.
