骑缝钉在小儿下颌骨创伤的应用山西省荣军医院(030031)曹向山1986~1990年我院口腔颌面外科用记忆合金压缩骑缝钉治疗下颌骨创伤患儿3例,取得满意效果,现介绍如下:临床资料男2例,女1例;分别为6岁、7岁和10岁,3例患儿均是下颌骨创伤骨折,骨折部位分别为左下颌体,正中联合和左下颌角,其中左下颌角骨折为粉碎性,其余两例为横断骨折。3例均有明显错位,错畸形。创伤时间分别为2天、8天和36天,其中36天的一例已畸形愈合。手术方法:全麻下清创完毕后,暴露骨折断端。畸形愈合的首先切开皮肤、皮下、骨膜、分离、去掉骨痂,将骨折断端复位,对位。在断端两侧骨组织上,与骨表面成垂直方向各钻两个小孔,一般小儿用1.2×10#的骑缝钉2枚,将刺钉插入骨孔内,将切口缝合。术后4周取出骑缝钉,取钉时只暴露臂杆,用小剥离子撬出即可。结果:3例患儿去除骑缝钉后,X丝检查骨折部位对位良好,胃质愈合,开口正常,咬关系正常。讨论骑缝钉是用具有特异功能的金属——镍钛形记忆合金制成,具有特殊的物理特性,即它在温度降低后变软可作任意塑形,温度升高后它有精确记忆力而恢复到原来的形状,在恢复原形的过程中产生相当大的纵向收缩力,这种机械力在骨折断端
