颞叶癫痫(TLE)为最常见的一类部分性癫痫。目前对于药物难治性颞叶癫痫的治疗主要靠外科手术,但癫痫病因复杂,种类繁多,手术治疗难度极大。而颞叶癫痫的形态学改变能从微观到宏观,更好的解释颞叶癫痫的形成机制,指导手术的定位及切除范围。随着新型成像技术的出现及癫痫病灶超微病理的深入研究,癫痫的手术疗效逐渐提高。本文综述了颞叶癫痫病灶的影像学及光镜、电镜下形态学改变及其与外科手术的关系。
Temporal lobe epilepsy (TLE) is the most common type of partial epilepsy. Currently for the treatment of refractory temporal lobe epilepsy mainly by surgical treatment, but the etiology of epilepsy, a wide range of surgical treatment is extremely difficult. The morphological changes of temporal lobe epilepsy can be microscopic to macroscopic, better explain the temporal lobe epilepsy formation mechanism, guide the operation of the positioning and resection range. With the advent of new imaging techniques and the further study of epileptic foci ultrastructural pathology, the efficacy of epilepsy surgery gradually increased. This article reviews the imaging of temporal lobe epilepsy lesions and morphological changes under light and electron microscopes and their relationship with surgical procedures.
