目的探讨肥胖孕妇外周血中肥胖抑制素水平与胰岛素抵抗之间的相关性。方法选取2011年1月—2013年12月于武警后勤学院附属医院门诊产检的肥胖孕妇30例作为肥胖组,取同期正常体重孕妇35例作为对照组,于孕中期抽取外周血检测相关指标。结果与对照组比较,肥胖组孕妇血清总胆固醇、甘油三酯水平增高,差异有统计学意义(P<0.05);与对照组孕妇比较,肥胖组空腹血糖和胰岛素水平均增高,胰岛素抵抗指数(HOMA-RI)增高,差异均有统计学意义(P<0.05);与对照组比较,肥胖组孕妇外周血中肥胖抑制素水平降低,差异有统计学意义(P<0.05);肥胖组孕妇外周血中肥胖抑制素水平与空腹血糖、空腹胰岛素、HOMA-RI呈负相关(r值分别为-0.898,-0.840,-0.789)。结论肥胖孕妇存在肥胖抑制素降低,以及脂和糖代谢异常,肥胖抑制素可能是影响胰岛素抵抗的重要因素。
Objective To investigate the correlation between obesity statin levels and insulin resistance in obese pregnant women. Methods From January 2011 to December 2013, 30 obese pregnant women who were diagnosed in the outpatient service of Affiliated Hospital of Armed Police Forces Hospital were selected as the obese group. 35 pregnant women with normal body weight were selected as the control group. The relevant indexes of peripheral blood were collected in the second trimester. Results Compared with the control group, the levels of serum total cholesterol and triglyceride in obese group were significantly higher than those in control group (P <0.05). Compared with pregnant women in control group, the fasting blood glucose and insulin levels were increased in obese group. The insulin resistance index (P <0.05). Compared with the control group, the level of obesity inhibin in pregnant women in obese group was lower than that in control group (P <0.05), and the difference was statistically significant The level of obesity inhibin was negatively correlated with fasting plasma glucose, fasting insulin and HOMA-RI (r = -0.898, -0.840, -0.789, respectively). Conclusion obese obesity decreased fat obesity, as well as abnormal lipid and glucose metabolism, obesity inhibin may be an important factor affecting insulin resistance.
