目的 :分析高场强下原发性肝癌的MRI表现。方法 :收集经手术或尸检病理证实的原发性肝癌MR资料 43例 ,其中巨块型肝癌 32例 ,其中伴子灶 3例 ,结节型 5例 ,小肝癌 6例。设备为GE 1.5T超导signaadvantageMR。应用SET1WI ,FSEPDWI ,T2 WI进行轴位扫描 ,所有病例均行增强扫描。结果 :肝癌在T1WI呈高、等、低信号。在T2 WI呈高信号 ,部分肿瘤内部信号不均 ,肝癌的强化程度主要取决于肿瘤血供的多少 ,肝癌的包膜由纤维组织构成 ,表现为低信号环 ,无包膜的肿瘤门静脉侵犯发生率高 ,肿瘤的信号及包膜与其病理分级有一定的相关性。结论 :MRI对反映原发性肝癌的病理特征有其独特的优越性。
Objective: To analyze the MRI features of primary liver cancer under high field strength. Methods: 43 cases of primary hepatic carcinoma confirmed by surgery or autopsy were collected. Among them, there were 32 cases of massive hepatocellular carcinoma, including 3 cases of companion tumor, 5 cases of nodular type, and 6 cases of small hepatocellular carcinoma. The equipment is a GE 1.5T superconducting signaadvantage MR. Axial scans were performed using SET1WI, FSEPDWI, and T2WI, and all patients underwent enhanced scans. Results : Liver cancer showed high, equal and low signal on T1WI. High signal intensity on T2 WI, partial intratumoral signal inhomogeneity, liver cancer enhancement depends on the amount of tumor blood supply, liver cancer capsule is composed of fibrous tissue, manifested as a low signal ring, non-enveloped tumor portal vein invasion occurred The rate is high, and there is a certain correlation between the tumor signal and the envelope and its pathological grade. Conclusion: MRI has unique advantages in reflecting the pathological features of primary liver cancer.
