目的观察重度新生儿缺氧缺血性脑病 (HIE)患儿脑干听觉诱发电位变化并探讨其与后期治疗的关系。方法对 13 2例重度 HIE患儿治疗 2周时进行脑干听觉诱发电位 (BAEP)检测 ,61例异常者于 6个月龄时复查 BAEP;治疗组 45例给予脑活素、复方丹参多疗程治疗及听觉刺激 ,16例家长放弃后期治疗者为对照组。结果治疗 2周时 BAEP检测异常 65例 ,异常率为 49.2 4%。 6个月龄时复查BAEP,治疗组恢复率为 93 .3 3 % ,对照组为 68.75 % ,两组比较有显著性差异 (χ2 =4.8,P<0 .0 5 )。结论重度 HIE患儿听力损害发生率高 ,对其进行 BAEP检测很有必要 ;新生儿期后脑活素、复方丹参多疗程治疗及听觉刺激干预 ,可促使听力恢复 ,减少伤残率。
Objective To observe the changes of brainstem auditory evoked potentials in children with severe neonatal hypoxic-ischemic encephalopathy (HIE) and to explore the relationship between them and the later treatment. Methods The brainstem auditory evoked potentials (BAEP) were detected in 132 children with severe HIE at 2 weeks. The BAEP was examined in 61 children with abnormalities at 6 months of age. The patients in the treatment group were treated with cerebrolysin, Treatment and auditory stimulation, 16 cases of parents give up treatment for the control group. Results Two weeks after treatment BAEP abnormalities in 65 cases, the abnormal rate was 49.2 4%. The BAEP was reviewed at 6 months of age. The recovery rate was 93.33% in the treatment group and 68.75% in the control group. There was significant difference between the two groups (χ2 = 4.8, P <0.05). Conclusions The incidence of hearing loss in children with severe HIE is high, and it is necessary to detect BAEP. The treatment of cerebrolysin and compound Salvia miltiorrhiza during the neonatal period and hearing stimulation can promote the hearing recovery and reduce the disability rate.
