目的观察重组人表皮生长因子(rhEGF)对持续性高眼压致角膜内皮损伤修复的影响。方法24只成年家兔,前房注入1%甲基纤维素0.20 ml建立眼压30~40 mmHg、持续1~2周的慢性高眼压动物模型。每只兔双眼随机分为rhEGF治疗组和对照组。治疗组局部滴用100μg/ml rhEGF 4次/d,对照组点用赋型剂。在术前及术后第1、3、7天分别检测角膜内皮细胞密度(ECD)、内皮细胞面积变异系数(CV)、六边形细胞百分数(6A)和中央角膜厚度(CCT),采用透射电镜(TEM)观察角膜超微病理改变。结果术后第1天rhEGF治疗组ECD、6A及CCT与对照组比较,差异均无显著性意义(均P>0.05)。术后第3、7天,治疗组ECD、6A较对照组明显增高,CV比对照组低,CCT较对照组薄,差异均有显著性意义(P<0.05或P<0.01)。术后第7天透射电镜检查显示治疗组再生的角膜内皮细胞可见更多的粗面内质网和核糖体。结论局部用rhEGF可有效促进持续性高眼压致兔角膜内皮损伤的修复。
Objective To observe the effect of recombinant human epidermal growth factor (rhEGF) on the repair of corneal endothelium injury induced by continuous ocular hypertension. Methods Twenty-four adult rabbits were injected with 0.20 ml methylcellulose (1%) in the anterior chamber to establish a chronic ocular hypertension model with intraocular pressure (IOP) of 30-40 mmHg for 1-2 weeks. Each rabbit eyes were randomly divided into rhEGF treatment group and control group. The treatment group local drip with 100μg / ml rhEGF 4 times / d, the control group with excipients. Corneal endothelial cell density (ECD), coefficient of variation of endothelial cell area (CV), percentage of hexagonal cells (6A) and central corneal thickness (CCT) were measured before operation and on the 1st, Electron microscopy (TEM) observed corneal ultrastructure pathological changes. Results There was no significant difference in the ECD, 6A and CCT levels between the rhEGF treatment group and the control group on the first postoperative day (all P> 0.05). At 3 and 7 days after operation, the ECD and 6A in the treatment group were significantly higher than those in the control group, CV was lower than the control group, and CCT was thinner than the control group (P <0.05 or P <0.01). Transmission electron microscopy on the 7th postoperative day revealed more rough endoplasmic reticulum and ribosomes in the treated corneal endothelial cells. Conclusion Topical application of rhEGF can effectively promote the repair of corneal endothelial cells in rabbits with persistent ocular hypertension.
