目的探讨16层螺旋CTA对主动脉夹层动脉瘤的诊断价值。方法使用16层螺旋CT对20例主动脉夹层(AD)患者行MSCTA检查。结果 20例中,4例为DeBakey I型,3例为DeBakerlⅡ型,其余13例为DeBakeyⅢ型。所有病例均很好地显示了主动脉全程及其分支、真腔、假腔、内膜片及夹层动脉瘤的部位、范围、附壁血栓、管壁钙化情况和病变空间关系。结论 MSCT多维重建能更清晰地显示主动脉弓夹层的直接、间接征象,了解主动脉弓血管受累情况,对临床制定合理的治疗方案提供重要信息,并可作为主动脉弓夹层动脉瘤的首选的诊断及随访检查方法之一。
Objective To investigate the diagnostic value of 16-slice spiral CTA for aortic dissection aneurysm. Methods Twenty patients with aortic dissection (AD) were examined by MSCTA with 16-slice spiral CT. Results Of 20 cases, 4 were DeBakey type I, 3 were DeBakerl type II and the remaining 13 were DeBakey type III. In all cases, the location and extent of the entire aorta and its branches, true lumen, false lumen, endomembrane and dissecting aneurysms were well displayed in all cases. The thrombosis of the wall, the calcification of the wall, and the spatial relationship of the lesions were also observed. Conclusion The MSCT multi-dimensional reconstruction can show the direct and indirect signs of aortic arch dissection more clearly and understand the vascular involvement of the aortic arch. It can provide important information for establishing a reasonable clinical treatment plan and can be used as the first choice of diagnosis and follow-up examination of aortic arch dissecting aneurysms one.
