1992年3月20日,四川省威远钢铁厂1号高炉(87m~3),在富氧鼓风试验送氧调试中开启氧气总阀时发生剧烈爆炸,致使在场人员全部受害(死亡14人,重伤3人,轻伤6人),炸毁氧气总阀和阀后近1m的不锈钢管。造成这次事故的原因主要是: (1)开启氧气总阀时,阀前与阀后压差过大(阀前压力2MPa,阀后为0.1MPa),违反了冶金部《氧气安全规定》中“直径大于70mm的手动氧气阀门,只有阀门前后压差小于0.3MPa才允许操作”的规定。 (2)由于压差过大,开启阀门时氧气流速极高,加之新安装的管道内存在铁屑、焊渣等杂物,又无接地装置,管道脱脂也未
