目的:比较慢性鼻-鼻窦炎伴鼻息肉(CRSwNP)及慢性鼻-鼻窦炎不伴鼻息肉(CRSsNP)患者鼻腔金黄色葡萄球菌携带毒力基因的差异,探索不同毒力因子与鼻窦炎、鼻息肉发病之间的关系。方法:以71例CRS患者为实验组(CRSwNP患者33例,CRSsNP患者38例),同期103例非CRS者为对照组。采用PCR方法对从鼻腔内分泌物中分离鉴定的41株金葡菌所携带的17种毒力基因进行检测。结果:①实验组seb检出率明显高于对照组(χ~2=5.864,P<0.05);CRSwNP组中seb检出率高于CRSsNP组(P<0.05);其余5种肠毒素基因实验组与对照组比较均差异无统计学意义(P>0.05);②金葡菌黏附素在实验组及对照组中均有检出,且检出率都较高,比较实验组和对照组检出率时,差异无统计学意义(P>0.05);③编码溶血素的HLα、HLβ基因在实验组和对照组中检出率都较高,且这两种基因在实验组与对照组中比较均差异无统计学意义(P>0.05)。结论:携带有肠毒素基因的金葡菌更易引起CRS,且可能携带肠毒素种类越多的菌株感染与鼻息肉的发病关系越大。
OBJECTIVE: To compare the differences in the virulence genes of nasal Staphylococcus aureus among patients with chronic rhinosinusitis and nasal polyps (CRSwNP) and chronic rhinosinusitis without nasal polyps (CRSsNP) and to explore the relationship between different virulence factors and sinusitis, nasal The relationship between the incidence of polyps. Methods: Totally 71 patients with CRS were selected as experimental group (33 patients with CRSwNP and 38 patients with CRSsNP) and 103 non-CRS patients as control group. CONCLUSIONS: Staphylococcus aureus carrying enterotoxin gene is more likely to cause CRS, and the more bacterial infection may be associated with the pathogenesis of nasal polyps.
