探讨微创通道经皮肾镜结合输尿管软镜钬激光碎石术治疗肾下盏结石的疗效及安全性。方法:分析38例肾脏下盏结石的临床资料,采用G20通道经皮肾镜在B超定位下,选择肾脏中盏穿刺通道结合输尿管软镜钬激光碎石术治疗肾下盏结石38例。结果:38例肾下盏结石患者平均手术治疗时间为2.28h,住院天数为11.5d。术后复查KUB平片,33例患者一期排净结石,结石清除率为86.8%,其中2例患者因通道不成功改二期手术,3例患者有少量残石,带双J管行ESWL,无严重并发症发生。结论:G20微创通道经皮肾镜结合输尿管软镜钬激光碎石术是治疗肾脏下盏结石合适的选择,能安全有效提高一期手术成功率及结石清除率。
To evaluate the efficacy and safety of minimally invasive percutaneous nephrolithotomy combined with ureteroscopic holmium laser lithotripsy in the treatment of lower calyceal calculus. Methods: The clinical data of 38 cases of inferior calyceal calculus were analyzed. 38 cases of lower calyceal calculus were treated by middle renal puncture channel and ureteroscopic holmium laser lithotripsy under G-channel percutaneous nephrolithotomy. Results: The average operation time of 38 patients with lower calyx stones was 2.28 hours and the length of stay was 11.5 days. Postoperative KUB plain film, 33 patients with a row of clear stones, stone clearance rate was 86.8%, of which 2 patients due to unsuccessful changes to the second phase of surgery, 3 patients with a small amount of residual stone, with double J tube ESWL No serious complications occurred. Conclusion: G20 minimally invasive percutaneous nephrolithotomy combined with ureteroscopic holmium laser lithotripsy is an appropriate choice for the treatment of lower kidney stones. It can safely and effectively improve the success rate of primary surgery and stone clearance rate.
