目的:探讨心腔声学造影时间-强度曲线(TIC)参数定量评价右心收缩功能的可靠性及寻找反映右心收缩功能的可靠指标。方法:10只犬分别在基础状态及注射心得安后于心功能减退状态下经股静脉重复注射5%声振清蛋白注射液,剂量0.08ml/kg,选取心尖四腔切面观察右心室造影剂的充盈及清除。采用拟合曲线定量分析方法获取心腔声学造影TIC参数,并分析参数在不同的心功能水平的差异。结果:随心功能的减退,时间常数(k)明显减小,而造影剂峰值强度减半时间(HT)及造影剂平均通过时间(MTT)明显延长(P<0·01)。k、HT及MTT分别与右心排血量、左室射血分数左室短轴缩短率明显相关。结论:采用拟合曲线定量分析技术获得右心腔声学造影TIC,用于定量估测右心收缩功能是可靠的,其中k、HT和MTT是反映右心收缩功能的可靠指标。
Objective: To evaluate the reliability of quantitatively evaluating right ventricular systolic function (TIC) parameters and to find a reliable index to reflect right ventricular systolic function. Methods: Ten dogs were injected with 5% sonicated albumin (0.08ml / kg) through the femoral vein repeatedly in the state of basal state and after injection of propofol. The fitting curve quantitative analysis method was used to obtain the TIC parameters of cardiac chamber and the differences of parameters in different cardiac function levels were analyzed. Results: The time constant (k) decreased significantly with the decrease of cardiac function, while the half time of peak intensity of contrast medium (HT) and the mean transit time (MTT) of contrast medium significantly prolonged (P <0.01). k, HT and MTT were significantly correlated with right ventricular ejection fraction, left ventricular ejection fraction shortening of left ventricular short axis. CONCLUSION: TIC obtained by quantitative curve fitting technique is reliable for quantitative assessment of right ventricular systolic function. Among them, k, HT and MTT are reliable indicators of right ventricular systolic function.
