目的调查海南省人民医院门诊处方用药情况,通过处方点评,分析不合理处方原因,为处方管理及干预提供依据,促进临床合理用药。方法随机抽取2013年6月至11月海南省人民医院门诊处方共720张,依据《处方管理办法》、《医院处方点评管理规范(试行)》、《抗菌药物临床应用指导原则》、药品说明书及海南省人民医院处方点评制度等进行逐张审核,并对结果进行统计分析。结果每张处方平均药品品种数为2.12,平均处方金额为184.76元,抗菌药物使用率为27.50%,注射剂使用率为22.08%,国家基本药物使用率为31.07%,药品通用名使用率为97.83%。处方合格率92.64%,不合理处方主要表现为不规范处方和不适宜处方。结论海南省人民医院门诊仍存在不合格处方和不合理用药现象,应持续开展处方点评工作,加强医务人员各项规章制度及专业知识的培训,提高临床合理用药水平。
Objective To investigate the prescription medication in outpatient department of Hainan Provincial People’s Hospital and to provide prescriptions for prescription management and intervention through prescription reviews and analysis of reasons for unreasonable prescriptions to promote clinical rational drug use. Methods A total of 720 outpatient prescriptions were randomly selected from Hainan Provincial People’s Hospital from June to November 2013. According to the “prescription management approach”, “hospital prescription reviews management practices (trial)”, “guidelines for the clinical application of antimicrobial agents”, drug specification and Hainan Provincial People’s Hospital prescription review system, etc. one by one audit, and the results of statistical analysis. Results The average number of drugs per prescription was 2.12, the average prescription value was 184.76 yuan, the antibacterial drug usage rate was 27.50%, the injection rate was 22.08%, the national basic drug usage rate was 31.07% and the generic drug usage rate was 97.83% . Prescription pass rate of 92.64%, unreasonable prescription mainly for non-standard prescription and not suitable for prescription. Conclusion There are still unqualified prescriptions and irrational drug use in outpatient clinics of Hainan Provincial People’s Hospital. We should continue to carry out prescription review work, strengthen the training of medical staffs on rules, regulations and professional knowledge, and raise the level of clinical reasonable medication.
