目的探讨彩色多普勒超声(US)与磁共振成像(MRI)在乳腺癌诊断中的应用价值。方法选取湖北省荆州市中心医院2012年2月至2014年5月接受US及MRI检查的430例(501个病灶)乳腺病变患者的临床资料,对照病理诊断结果,比较US、MRI以及两者联合应用对乳腺癌诊断的符合率。结果 501个病灶中,US对良性病灶的诊断准确率为79.6%(187/235),对恶性病灶的诊断准确率为83.1%(221/266)。MRI对良性病灶的诊断准确率为87.0%(200/230),对恶性病灶的诊断准确率为90.0%(244/271)。MRI对乳腺病变的诊断符合率高于US,但两组比较差异无统计学意义(P>0.05)。US联合MRI诊断对乳腺良恶性病变的诊断符合率均高于单独采用US检查,组间比较差异有统计学意义(P<0.05)。结论 US及MRI对乳腺癌的诊断均具有一定的误诊率、漏诊率,US联合MRI诊断可提高乳腺癌诊断的准确率,可为乳腺癌的早期确诊提供参考,值得临床深入研究。
Objective To explore the value of color Doppler ultrasound (US) and magnetic resonance imaging (MRI) in the diagnosis of breast cancer. Methods The clinical data of 430 patients (501 lesions) with breast lesions who underwent US and MRI examinations from February 2012 to May 2014 in Jingzhou Central Hospital of Hubei Province were selected. The results of pathological diagnosis were compared with those of US, MRI and the combination of the two Application of diagnostic accuracy of breast cancer. Results Of the 501 lesions, the diagnostic accuracy of US was 79.6% (187/235) for benign lesions and 83.1% (221/266) for malignant lesions. The diagnostic accuracy of MRI for benign lesions was 87.0% (200/230), and the diagnostic accuracy of malignant lesions was 90.0% (244/271). The coincidence rate of MRI diagnosis of breast lesions was higher than that of US, but there was no significant difference between the two groups (P> 0.05). The coincidence rate of US joint MRI diagnosis of benign and malignant breast lesions was higher than that of US alone. The difference between the two groups was statistically significant (P <0.05). Conclusions Both US and MRI have some misdiagnosis rates for breast cancer diagnosis. Misdiagnosis rate and US joint MRI diagnosis may improve the accuracy of breast cancer diagnosis. It may provide a reference for early diagnosis of breast cancer and deserve further clinical study.
