角膜上皮下混浊(haze)和屈光回退是准分子激光屈光性角膜切削术(PRK)后的两个最常见和严重的并发症。羊膜曾被广泛地用于眼表重建,并且疗效显著;近几年来,在一系列动物实验中表明羊膜能减少haze的形成。它能减少PRK后早期角膜炎症反应,保护角膜细胞免受凋亡;在晚期修复反应中它能减少活化角膜基质细胞的增生,减少细胞外基质的合成和分泌。这样,羊膜通过调节haze形成和屈光回退的病理生理过程来减轻haze和屈光回退的发生,将其用于人类提高PRK疗效显示出有希望的前景。
Corneal epithelial haze and refractive regression are the two most common and serious complications after excimer laser refractive keratectomy (PRK). Amniotic membranes have been widely used in the reconstruction of ocular surface, and the curative effect is remarkable. In a series of animal experiments in recent years, it has been shown that amniotic membrane can reduce the formation of haze. It reduces early post-PRK corneal inflammatory response and protects corneal cells from apoptosis; it reduces the activation of corneal stromal cells and reduces extracellular matrix synthesis and secretion during late repair reactions. In this way, amniotic membrane haze and refractive regression through the regulation of haze formation and refractive regression of the pathophysiological process, its use in humans to improve the efficacy of PRK showed promising prospects.
