背景:国内外研究证实Twin-block矫治器不仅能有效地改变硬组织,还能有效地改变软组织侧貌,那么在软硬组织的改变间是否存在一定的关系和规律呢?目的:评价分析Twin-block功能矫治前后面型突度软硬组织的改变,并与自然生长组面型突度的改变进行对比。方法:选择Hagg手腕骨片为FG-G期的安氏Ⅱ类1分类下颌后缩患者31例,其中以接受Twin-block功能矫治的Ⅱ类错患者17例作为实验组,以放弃正畸治疗的Ⅱ类错患者14例为对照组,分别在治疗前后拍摄头颅侧位片。比较Twin-block功能矫治前后和自然生长前后侧位片面型突度软硬组织的改变。应用无常量的-元线性回归方程,分析软硬组织改变量的线性关系,并对Twin-block组和自然生长组的数学回归方程进行对比分析。结果与结论:发现在Twin-block治疗前后,上下齿槽座相对于鼻、颏位置的改变与上下唇凹点相对于鼻、颏位置的改变具有显著相关性,即A-E改变量与Ss-E改变量、B-E改变量与Si-E改变量,具有线性相关性,其中Si-E改变量与B-E改变量相关系数Pearson=0.839最大,说明二者的相关程度最好。对照组2对标志点无线性关系。通过统计分析得到Twin-block治疗前后面型突度软硬组织改变量关系的线性方程:Si-E=0.745B-E,Ss-E=0.276A-E。线性方程不仅有助于解释Twin-block矫治促进下颌骨生长的同时,安氏II类1分类下颌后缩患者的面型突度也随之发生明显的临床观察结果,而且有助于预测Twin-block矫治器面型突度的改变。
BACKGROUND: Domestic and international studies have confirmed that Twin-block appliance not only can effectively change hard tissue, but also can effectively change the soft tissue profile, then there is a certain relationship and law between the changes of soft and hard tissue.Objective: -block function before and after the treatment of the surface of the sudden change in the hardness of the soft and hard tissue, and with the natural growth of the surface of the type of sudden change in the contrast. Methods: Thirty-one patients with Class Ⅱ division 1 malocclusion of FG-G were selected from the Hagg wrist fragment. Seventeen patients with class Ⅱ malocclusion treated with Twin-block function were selected as the experimental group to abandon the orthodontic 14 patients with type Ⅱ malocclusion were treated as control group, and the cephalometric films were taken before and after treatment. Before and after Twin-block functional correction and before and after the natural growth of lateral flank soft and hard tissue changes in the organization. The linear regression equation was used to analyze the linear relationship between soft and hard tissue changes. The mathematical regression equations of Twin-block group and natural growth group were compared. RESULTS AND CONCLUSION: It was found that before and after Twin-block treatment, the position of upper and lower alveolar base relative to the nose and chin had a significant correlation with the change of the position of upper and lower lip pits relative to the nose and chin, namely, the change of AE and Ss-E The change amount, the BE change amount and the Si-E change amount have a linear correlation. The correlation coefficient between the Si-E change amount and the BE change amount is Pearson = 0.839, which shows that the correlation between them is the best. Control 2 had no linear relationship with the marker points. By statistical analysis, the linear equations of the relationship between the change of the hard and soft tissue of the surface type before and after the Twin-block treatment were obtained: Si-E = 0.745B-E and Ss-E = 0.276A-E. The linear equation not only helps to explain that Twin-block correction can promote the growth of the mandible, but also has obvious clinical findings along with the Class II Class 1 classification of mandibular retrusion patients. It is also helpful to predict the Twin- block appliance surface type change in the degree of projection.
