目的探讨CCR7在干燥综合征(SS)患者外周血T淋巴细胞的表达与疾病活动性的关系。方法选择31例干燥综合征患者,其中原发干燥综合征(pSS)18例,继发干燥综合征(sSS)13例,正常对照组20名,为健康体检者,应用流式细胞术检测外周血T淋巴细胞CCR7的表达,分析其与临床各指标的关系。结果①pSS和sSS组CD4+T细胞和CD8+T细胞上CCR7的表达均高于正常对照组(P<0.01);pSS组CD4+T细胞上CCR7的表达高于sSS组(P<0.05);此外,pSS组CCR7在CD4+T细胞上的表达明显高于同组的CD8+T细胞(P<0.01)。②pSS患者CD4+T细胞和CD8+T细胞上CCR7的表达均与血沉、血清IgG和血清γ球蛋白呈正相关,但与唾液腺、泪腺的排泌功能无相关性,而sSS患者CD4+T细胞、CD8+T细胞上CCR7的表达与临床各指标均无相关性。结论 SS患者T淋巴细胞表达CCR7明显增加,特别是pSS患者CCR7+CD4+T细胞增加尤为突出,表明CCR7可能参与了SS的发病和外分泌腺的病理损害,且pSS患者T细胞上CCR7的表达与ESR、血清IgG和血清γ球蛋白呈正相关,提示CCR7可用来作为临床上判定pSS的疾病活动性指标之一。
Objective To investigate the relationship between CCR7 expression in peripheral blood T lymphocytes and disease activity in patients with Sjogren’s syndrome (SS). Methods Thirty-one patients with Sjogren’s syndrome were selected, including 18 patients with primary sjogren syndrome (SSS), 13 patients with secondary sj-s syndrome (SSS) and 20 healthy controls. For healthy subjects, flow cytometry was used to detect the peripheral The expression of CCR7 in blood T lymphocytes was analyzed, and its relationship with clinical indicators was analyzed. Results ① The expression of CCR7 on CD4 + T cells and CD8 + T cells in pSS and sSS groups was higher than that in normal control group (P <0.01). The expression of CCR7 on CD4 + T cells in pSS group was higher than that in sSS group (P <0.05). In addition, the expression of CCR7 on CD4 + T cells in pSS group was significantly higher than that in CD8 + T cells in the same group (P <0.01). ② The expression of CCR7 in both CD4 + T cells and CD8 + T cells of patients with pSS was positively correlated with ESR, serum IgG and serum gamma globulin but not with the secretion of salivary gland and lacrimal gland. However, the expression of CD4 + T cells, The expression of CCR7 on CD8 + T cells was not correlated with clinical indexes. Conclusions The expression of CCR7 in T lymphocytes of patients with SS was significantly increased, especially the increase of CCR7 + CD4 + T cells in patients with pSS, which indicated that CCR7 may be involved in the pathogenesis of SS and pathological damage of exocrine glands. The expression of CCR7 on T lymphocytes in patients with SS ESR, serum IgG and serum gamma globulin were positively correlated, suggesting that CCR7 can be used as a clinical indicator of pSS disease activity.
