目的观察小剂量地西泮联合复方丹参滴丸治疗老年冠心病心绞痛对患者血小板活化及血管功能的影响。方法将208例老年冠心病心绞痛患者随机分为观察组和对照组各104例。对照组在常规治疗的基础上加用复方丹参滴丸,观察组在对照组治疗的基础上加用小剂量地西泮治疗。分别测定治疗前(T1)、治疗后7d(T2)、治疗后14d(T3)2组患者血小板活化及血管功能相关指标,比较两种治疗方案各项指标的差异。结果观察组T2、T3时血小板颗粒膜蛋白(CD63)、α-溶酶体膜蛋白(CD62p)、凝血酶敏感蛋白(TSP)、血小板膜糖蛋白Ⅱb~Ⅲa(PAC-1)表达率均低于对照组,血小板膜糖蛋白(CD42b)表达率高于对照组,差异均有统计学意义(P<0.05)。观察组T2、T3时内皮素(ET)和血栓素A2(TXA2)的表达水平均低于对照组,一氧化氮(NO)、一氧化氮合成酶(NOS)、前列环素(PGI2)的水平均高于对照组,差异均有统计学意义(P<0.05)。结论小剂量地西泮联合复方丹参滴丸干预治疗冠心病心绞痛,可明显抑制血小板活化的表达及改善血管功能障碍的状态,利于缓解心绞痛症状和提高治疗效果。
Objective To observe the effects of low-dose diazepam combined with compound Danshen dripping pills on platelet activation and vascular function in elderly patients with coronary heart disease and angina pectoris. Methods 208 elderly patients with coronary heart disease angina were randomly divided into observation group and control group of 104 cases. The control group was treated with compound Danshen dripping pills on the basis of routine treatment. The observation group was treated with low-dose diazepam on the basis of the control group. The indexes of platelet activation and vascular function in the two groups before treatment (T1), 7d (T2) after treatment and 14d (T3) after treatment were measured respectively. The differences between the two treatment regimens were compared. Results The expression rates of CD63, CD62p, TSP and PAC-1 in the observation group at T2 and T3 were both low In the control group, the expression of platelet membrane glycoprotein (CD42b) was higher than that of the control group, the difference was statistically significant (P <0.05). The levels of endothelin (ET) and thromboxane A2 (TXA2) in T2 and T3 in observation group were lower than those in control group, nitric oxide (NO), nitric oxide synthase (NOS) and prostacyclin (PGI2) The levels were higher than the control group, the differences were statistically significant (P <0.05). Conclusion Low-dose diazepam combined with compound Danshen dripping pills in the treatment of coronary heart disease angina pectoris can significantly inhibit the expression of platelet activation and improve the state of vascular dysfunction, which will help relieve the symptoms of angina pectoris and improve the therapeutic effect.
