目的观察Survivin与nm23-H1蛋白在大肠癌及其癌旁组织中的表达情况,探讨其联合检测与淋巴结转移的关系。方法采用免疫组化S-P法研究66例大肠癌和15例癌旁组织的Survivin和nm23-H1的表达情况,分析其与临床病理特征的关系。结果在大肠癌及其癌旁组织中,Survivin阳性率分别为75.7%、62.1%,有显著性差异(χ2=13.254,P=0.000);nm23-H1的阳性率分别为26.7%、93.3%,差异有统计学意义(χ2=4.125,P=0.042)。Survivin的表达与肿瘤分化程度、淋巴结转移、肝转移、Dukes分期有关(P<0.05)。nm23-H1的表达与淋巴结转移、肝转移、Dukes分期有关(P<0.01)。Survivin阳性率与淋巴结转移呈正相关(r=0.247,P=0.046),nm23-H1阳性率与淋巴结转移呈负相关(r=-0.359,P=0.003)。Survivin与nm23-H1蛋白表达呈负相关(r=-0.296,P=0.016)。结论 Survivin蛋白的抑制凋亡作用和nm23-H1蛋白的抑制转移作用可能在大肠癌的发生、发展过程中起重要作用,联合检测是判断大肠癌患者淋巴结转移倾向及预后的重要指标。
Objective To observe the expression of Survivin and nm23-H1 protein in colorectal carcinoma and its adjacent tissues and to explore the relationship between Survivin and nm23-H1 protein in lymph node metastasis. Methods The expressions of Survivin and nm23-H1 in 66 cases of colorectal carcinoma and 15 cases of paracancerous tissues were studied by immunohistochemical S-P method. The relationship between Survivin and nm23-H1 was analyzed. Results The positive rates of Survivin in colorectal cancer and its adjacent tissues were 75.7% and 62.1%, respectively (χ2 = 13.254, P = 0.000). The positive rates of nm23-H1 were 26.7% and 93.3% The difference was statistically significant (χ2 = 4.125, P = 0.042). Survivin expression was correlated with tumor differentiation, lymph node metastasis, liver metastasis and Dukes stage (P <0.05). The expression of nm23-H1 was correlated with lymph node metastasis, liver metastasis and Dukes stage (P <0.01). The positive rate of Survivin was positively correlated with lymph node metastasis (r = 0.247, P = 0.046). The positive rate of nm23-H1 was negatively correlated with lymph node metastasis (r = -0.359, P = 0.003). Survivin and nm23-H1 protein expression was negatively correlated (r = -0.296, P = 0.016). Conclusion The inhibitory and apoptotic effects of Survivin and the metastasis of nm23-H1 protein may play an important role in the carcinogenesis and progression of colorectal cancer. Combined detection is an important index for judging the lymph node metastasis tendency and prognosis of patients with colorectal cancer.
