本文提出了左室功率(氧运输指数/左室作功)和右室功率(氧运输指数/右室作功)两个综合考核心肺功能的新指标。并根据11例自主呼吸和13例机械通气肺心病患者应用硝普钠和阿米托林后的血气和血流动力学改变,探讨了其临床应用价值。结果表明,硝普钠和阿米托林对血气和血流动力学均有明显影响,单用血气或血流动力学均不足以全面科学地评价其临床疗效。而心室功率则兼容了血气和血流动力学的变化,便于全面评价心肺功能,同时提出了慢阻肺患者血流动力学考核的三级标准。
In this paper, new indexes of cardiopulmonary function such as left ventricular power (oxygen transport index / left ventricular work) and right ventricular power (oxygen transport index / right ventricular work) are proposed. And based on 11 cases of spontaneous breathing and 13 cases of mechanical ventilation pulmonary heart disease patients with sodium nitroprusside and amiodarine after blood gas and hemodynamic changes to explore its clinical value. The results showed that both sodium nitroprusside and ammitriptan had significant effects on blood gas and hemodynamics, and blood gas or hemodynamics alone were not enough to evaluate the clinical effect comprehensively and scientifically. The ventricular power is compatible with changes in blood gas and hemodynamics, to facilitate a comprehensive evaluation of cardiopulmonary function, at the same time put forward three levels of hemodynamic assessment of patients with COPD.
