目的分析甲状腺微小癌的声像图及超声造影特点,总结高频超声结合超声造影对甲状腺微小癌的诊断价值。方法选择自2011年12月至2012年12月经手术病理证实的小于1.0 cm的甲状腺微小癌33例,分析甲状腺微小癌的声像图表现及超声造影特点。结果 33例经病理诊断为甲状腺癌的患者发现癌结节34个,超声诊断正确为30例,符合率为88.2%,甲状腺微小癌结节超声特点为:结节呈极低回声,边界模糊,无包膜,形态欠规则,沙砾样钙化,后方回声衰减,部分结节内部血流不丰富。超声造影特点:甲状腺微小癌的结节表现为不均匀弱增强,在整个增强过程中程度较正常甲状腺组织低。结论甲状腺微小癌的超声表现及造影具有典型特征,常规超声结合超声造影能够提高对甲状腺微小癌的诊断。
Objective To analyze the ultrasonographic features of thyroid microcarcinoma and summarize the diagnostic value of high frequency ultrasound combined with contrast-enhanced ultrasound in the diagnosis of thyroid microcarcinoma. Methods Thirty-three cases of thyroid microcarcinoma less than 1.0 cm proved by surgery and pathology from December 2011 to December 2012 were analyzed. The ultrasonographic features of the thyroid microcarcinoma were analyzed. Results Thirty-three patients with pathologically diagnosed thyroid cancer were diagnosed as having cancer nodules. The ultrasound was correctly diagnosed in 30 patients and the coincidence rate was 88.2%. The ultrasonographic features of the thyroid microcarcinoma were as follows: the nodules were extremely hypoechoic, Non-enveloped, irregular morphology, gravel-like calcification, attenuation of the rear echo, part of the nodules within the blood flow is not rich. Contrast characteristics of ultrasound: nodules of thyroid microcarcinoma showed uneven enhancement, in the whole process of enhancement lower than normal thyroid tissue. Conclusions Ultrasonographic features and contrast-enhanced imaging of thyroid microcarcinoma are typical features. Conventional ultrasound combined with contrast-enhanced ultrasound can improve the diagnosis of thyroid microcarcinoma.
