目的研究无创Bi-PAP呼吸机治疗慢性阻塞性肺疾病(COPD)合并Ⅱ型呼吸衰竭的临床意义。方法将93例COPD患者随机分为治疗组(51例),持续Bi-PAP呼吸机治疗;对照组(42例),持续低流量吸氧。两组同时进行抗感染等常规治疗。治疗前后观察血气和临床症状变化。结果治疗3日后治疗组比对照组其pH、PaCO2、PaO2明显恢复(P均<0.05)。结论COPD合并呼吸衰竭患者早期使用Bi-PAP呼吸机治疗,可明显改善低氧血症和CO2潴留,疗效显著。
Objective To study the clinical significance of noninvasive Bi-PAP ventilator in the treatment of chronic obstructive pulmonary disease (COPD) complicated with type Ⅱ respiratory failure. Methods Ninety-three patients with COPD were randomly divided into treatment group (n = 51) and continuous Bi-PAP ventilator treatment. In control group (n = 42), continuous low-flow oxygen inhalation was continued. At the same time, the two groups were treated routinely for anti-infection. Blood gas and clinical symptoms were observed before and after treatment. Results After 3 days of treatment, the pH, PaCO2 and PaO2 of the treatment group were significantly higher than those of the control group (all P <0.05). Conclusions The early use of Bi-PAP ventilator in patients with COPD complicated with respiratory failure can significantly improve hypoxemia and CO2 retention, with a significant effect.
