目的:研究妊娠高血压综合征(妊高征)患者胎盘组织中细胞凋亡及胰岛素样生长因子—I(Insulin-likegrowthfactor-I,IGF-I)表达水平的变化及IGFI与细胞凋亡的关系。方法:①采用流式细胞计量术检测52例妊高征患者胎盘组织标本(轻度16例,中度19例,重度17例)和30例正常妊娠胎盘组织标本的凋亡率;②应用免疫组织化学SP法对上述两组胎盘组织检测IGF-I的表达。结果:①妊高征组胎盘组织的凋亡率显著高于正常妊娠组(P<0.001);妊高征组中:中、重度妊高征显著高于轻度妊高征(P<0.01),重度妊高征显著高于中度妊高征(P<0.01);②IGF-I在妊高征组与正常妊娠组胎盘的合体滋养细胞、细胞滋养细胞及蜕膜细胞中均有表达,妊高征组均显著高于正常妊娠组(P<0.01),妊高征组间比较也有显著性差异(P<0.01);③妊高征组胎盘细胞凋亡率的增高与IGFI表达增高呈正相关。结论:妊高征与细胞凋亡增高有关,且妊高征病情随凋亡率的上升而加重。IGF-I的表达增加与妊高征的发生有关,与临床分度密切相关,即妊高征病情随IGF-I的上升而加重,可能通过对细胞凋亡的调节而对妊高征的演进过程产生影响。
AIM: To investigate the changes of apoptosis and the expression of insulin-like growth factor-I (IGF-I) in placental tissue and the relationship between IGFI and apoptosis in patients with pregnancy induced hypertension (PIH) . Methods ① Flow cytometry was used to detect the apoptosis rate of placenta from 52 patients with PIH (mild 16, moderate 19, severe 17) and 30 normal pregnancy The histochemical SP method was used to detect the expression of IGF-I in the two groups of placenta. Results: ①The apoptosis rate of placenta in PIH group was significantly higher than that in normal pregnancy group (P <0.001). In PIH group, the rates of moderate and severe PIH were significantly higher than those in mild PIH (P <0.01) , Severe PIH was significantly higher than that of PIH (P <0.01); ②IGF-I was expressed in syncytiotrophoblast, cytotrophoblast and decidual cells of placenta in PIH group and normal pregnancy group, (P <0.01). There was also a significant difference between the groups of PIH (P <0.01). ③The increase of placental cell apoptosis rate in PIH group was positively correlated with the increase of IGFI . Conclusion: PIH is associated with increased apoptosis, and PIH is aggravated with the increase of apoptosis rate. The increased expression of IGF-I is associated with the occurrence of PIH, which is closely related to the clinical grade, that is, the condition of pregnancy-induced hypertension aggravates with the increase of IGF-I, possibly through the regulation of apoptosis on the progression of PIH The process has an impact.
