目的探讨高频彩超在乳腺良恶性病变诊断中的应用价值。方法选取四平市妇婴医院2011年3月~2013年6月收治的乳腺肿块患者126例,患者行高频超声检查,观察肿瘤的边界、形态、内部回声及钙化情况,彩色多普勒测定肿块血流情况,并将超声诊断结果与病理确诊结果进行对比分析。结果病理结果显示良性肿块80例(63.5%),恶性肿块46例(36.5%);超声检查诊断良性肿块84例(66.7%),恶性肿块42例(33.3%),两种方法诊断结果差异无统计学意义(P>0.05)。乳腺恶性肿块超声声像图的典型表现为不规则形态,多数肿块无包膜或包膜不完整,边界呈毛刺状,肿块内见微小钙化点呈簇状分布;良性肿块超声声像图显示肿块形状规则,呈圆形或椭圆形,并且边界清晰,包膜完整,肿块内少有钙化灶或钙化点。良、恶性肿块组不规则形态、边界毛刺、不完整包膜、肿块内钙化、淋巴结肿大检出率比较,差异有统计学意义(P<0.05)。彩色多普勒超声检查显示恶性肿块最大血流速度、阻力指数均明显高于良性肿块组,差异有统计学意义(P<0.05)。结论高频彩超可有效显示乳腺肿块形态、边界、包膜、内部钙化及血流情况,有利于鉴别肿块性质,对临床诊断和治疗有重要指导意义。
Objective To investigate the value of high frequency ultrasound in the diagnosis of benign and malignant breast lesions. Methods 126 cases of breast masses were selected from MCH hospital of Siping City from March 2011 to June 2013. The patients were examined by high frequency ultrasound to observe the boundary, Blood flow, and ultrasound diagnostic results and pathological diagnosis results were compared. Results Eighty cases (63.5%) of malignant tumors and 46 (36.5%) of malignant tumors were found by pathology. There were 84 (66.7%) cases of benign mass and 42 (33.3%) of malignant masses diagnosed by ultrasonography. Statistical significance (P> 0.05). Malignant breast ultrasound sonography of the typical irregular shape, the majority of lumps without capsule or capsule is incomplete, the border was burr-like, the mass showed tiny calcification points were clustered distribution; benign mass ultrasound sonography showed mass The shape of the rules, was round or oval, and the border is clear, complete capsule, tumor calcification within the calcification or calcification. There were significant differences between the benign and malignant tumors in irregular shape, border burr, incomplete capsule, intraluminal calcification and lymphadenopathy (P <0.05). Color Doppler ultrasound showed that the maximum blood flow velocity and resistance index of malignant tumor were significantly higher than that of benign tumor group (P <0.05). Conclusion High-frequency color Doppler ultrasound can effectively display breast lump morphology, border, envelope, internal calcification and blood flow conditions, which is helpful to identify the nature of lumps and has important guiding significance for clinical diagnosis and treatment.
