患者,男,21岁,本院护士。反复面部肿痛,伴头痛3年,急性发作5天,于1973年7月16日入院。1970年发生左侧面部红肿痛,伴头痛,原因不明。经治疗后每年复发1～2次,此次发病3天后,门诊X线照片为“双侧上颌窦及筛窦感染”。检查:左侧面部肿胀、红热,双侧上颌窦前壁压痛。口腔45牙龈充血肿胀、触痛,腭扁桃体I°大,表面粗糙,隐窝内有豆渣样物存留,挤压痛。双侧颌下淋巴结鸽蛋大。血象:白细胞11,200。中性62％,淋巴24％,酸性细胞12％,大单核2％。出凝血时间各1分钟。诊断:①慢性上颌窦及筛窦炎急性发作;②慢性扁桃体炎;③45牙龈脓肿。
Patient, male, 21 years old, hospital nurse. Repeated facial swelling and pain, with headache 3 years, acute attack 5 days, July 16, 1973 admission. The left side of the face in 1970, swelling and pain, with headache, for unknown reasons. After treatment relapse 1 to 2 times a year, the incidence of 3 days after the clinic X-ray photo of “bilateral maxillary sinus and ethmoid infection.” Check: the left facial swelling, red hot, bilateral anterior sinus tenderness. Oral 45 gingival hyperemia, tenderness, I am large palatine tonsils, rough surface, crypts in the crypt retention, squeezing pain. Bilateral submandibular lymph nodes a large egg. Blood: white blood cells 11,200. Neutral 62%, lymphatic 24%, acidic cells 12%, large mononuclear 2%. A clotting time of 1 minute. Diagnosis: ① chronic maxillary sinus and ethmoid sinus attack; ② chronic tonsillitis; ③ 45 gingival abscess.
