目的评价实时二维超声技术在颅脑肿瘤外科手术中的应用价值。方法①28例颅脑肿瘤神经外科显微手术中应用实时超声技术,进行导航定位,选择皮层入路,实时指导肿瘤切除。②测量术中实时超声肿瘤体积并与术前MRI进行对比分析。结果①本组28例手术应用该技术定位精确,皮层切口减小,能清晰提供肿瘤动态切除图像;其中全切除25例(89.3%)、次全切除3例(10.7%);2例出现肌力短暂下降,2周后恢复至术前水平。②术中实时超声测得肿瘤体积值与术前MRI符合,P>0.05,差异无统计学意义。结论颅脑肿瘤手术中应用超声技术具有病灶定位快速准确、图像实时可靠等特点,通过术中动态扫描,有助于提高肿瘤的全切率和手术的安全性。
Objective To evaluate the value of real-time two-dimensional ultrasound in the surgical treatment of brain tumors. Methods ①28 cases of neurosurgery craniocerebral tumor microsurgery using real-time ultrasound technology, navigation and positioning, select the cortical approach, real-time guided tumor resection. ② measure intraoperative real-time ultrasound tumor volume and preoperative MRI comparative analysis. Results ①There were 28 cases (89.3%) with subtotal resection and 3 cases (10.7%) with subtotal resection and 2 cases with musculature A brief decrease in force, 2 weeks after the return to preoperative level. The intraoperative real-time ultrasound measured tumor volume values consistent with preoperative MRI, P> 0.05, the difference was not statistically significant. Conclusion The application of ultrasound technique in craniocerebral tumor surgery has the advantages of fast and accurate localization of the lesion, real-time reliable images and so on. Through dynamic intraoperative scanning, it helps to improve the rate of tumor resection and surgical safety.
