目的 探讨端粒酶作为大肠癌诊断、治疗和预后参数的价值。方法 应用PCR TRAP ELISA方法 ,检测大肠癌、癌旁和正常大肠粘膜组织的端粒酶活性表达。结果 端粒酶在大肠癌、癌旁和正常大肠粘膜组织中表达的阳性率分别为 84 8% (39/46 )、2 0 0 % (6 /30 )和 0 (0 /2 0 ) ,大肠癌组织中端粒酶表达阳性率明显高于癌旁和正常大肠粘膜(P <0 0 0 1)。早期大肠癌 (DukesA期 )即有 6 6 7%者存在端粒酶活化。肿瘤组织学分级越低 ,端粒酶表达阳性率越高(P <0 0 5 )。结论 端粒酶可能是大肠癌恶性浸润的早期事件 ,可作为诊断大肠癌的一个有用的辅助指标 ,并有助于预测预后和指导治疗方案的选择。
Objective To investigate the value of telomerase as a diagnostic, therapeutic and prognostic parameter for colorectal cancer. Methods The expression of telomerase activity in colorectal cancer, paracancerous and normal colorectal mucosa was detected by PCR TRAP ELISA. Results The positive rates of telomerase expression in colorectal cancer, paraneoplastic and normal colorectal mucosa tissues were 84 8% (39/46), 2 0 % (6/30) and 0 (0/20), respectively. The positive rate of telomerase expression in cancer tissue was significantly higher than that in the paracancerous and normal colorectal mucosa (P < 0.01). In early stage colorectal cancer (DukesA stage), telomerase activation was observed in 646% of patients. The lower the tumor histological grade, the higher the positive rate of telomerase expression (P < 0 05). Conclusion Telomerase may be an early event of malignant invasion of colorectal cancer and can be used as a useful auxiliary indicator for the diagnosis of colorectal cancer, and it can help predict prognosis and guide treatment options.
