目的探讨扩张型心肌病合并高血压患者的心功能及预后。方法选取2005—2010年我院收治的扩张型心肌病患者50例,根据患者病史中是否合并高血压将其分为合并高血压组27例和未合并高血压组23例。比较两组患者预后及心动图检查结果。结果两组患者病死率比较,差异无统计学意义(P>0.05)。合并高血压组患者室间隔厚度、左心室射血分数低于未合并高血压组,左心室后壁厚度及左心室心肌质量高于未合并高血压组,出院时美国纽约心脏病协会心功能分级(NYHA分级)改善程度优于未合并高血压组(P<0.05)。结论扩张型心肌病合并高血压患者室间隔厚度减小,左心室射血分数降低,心脏收缩功能增强,左心室心肌质量增大,但是运用血压水平作为排除高血压所引发心脏损害的手段具有一定的局限性。
Objective To investigate the cardiac function and prognosis of patients with dilated cardiomyopathy complicated with hypertension. Methods Fifty patients with dilated cardiomyopathy who were admitted to our hospital from 2005 to 2010 were divided into two groups according to their history of hypertension: 27 cases with hypertension and 23 cases without hypertension. The prognosis and the results of cardiogram were compared between the two groups. Results There was no significant difference in mortality between the two groups (P> 0.05). In patients with hypertension, the interventricular septum thickness, left ventricular ejection fraction were lower than those without hypertension, left ventricular posterior wall thickness and left ventricular myocardial mass were higher than those without hypertension. (NYHA classification) better than the non-hypertension group (P <0.05). Conclusions The patients with dilated cardiomyopathy complicated with hypertension have less ventricular septum thickness, decreased left ventricular ejection fraction, enhanced cardiac contractility, and increased left ventricular myocardial mass. However, the use of blood pressure as a means of excluding heart damage caused by hypertension Limitations.
