目的:探讨DOG1在胃肠道间质瘤(GIST)中的表达及其临床意义。方法:免疫组化Envision法对73例GIST和26例非GIST间叶源性肿瘤检测DOG1的表达,分析DOG1与GIST的各项临床病理参数之间的关系。结果:在73例GIST中DOG1、CD117和CD34的阳性率分别为91.78%、94.52%和91.78%,而在26例非GIST中阳性率为11.54%、3.85%和23.08%,以上指标在两组间的表达有统计学差异(P=0.000);DOG1检测GIST灵敏度和特异度分别为91.80%、88.50%,而DOG1联合CD117检测GIST灵敏度和特异度分别为98.60%和84.60%,灵敏度最高;DOG1的表达与GIST的性别、年龄、肿瘤的位置、肿瘤的大小、核分裂象和恶性潜能分级等临床病理因素无相关性(P>0.05),同时DOG1的表达与GIST的预后也无相关性(P>0.05)。结论:在GIST的诊断中DOG1与CD117一样是敏感而特异的指标,尤其是对CD117阴性的GIST的诊断中,能够起很好的补充作用,但不能作为评价GIST预后的指标。
Objective: To investigate the expression of DOG1 in gastrointestinal stromal tumors (GIST) and its clinical significance. Methods: The immunohistochemical Envision method was used to detect the expression of DOG1 in 73 cases of GIST and 26 cases of non-GIST mesenchymal tumor. The relationship between the clinicopathological parameters of DOG1 and GIST was analyzed. Results: The positive rates of DOG1, CD117 and CD34 in 91 cases of GIST were 91.78%, 94.52% and 91.78% respectively, while in 26 cases of non-GIST, the positive rates were 11.54%, 3.85% and 23.08% (P = 0.000). The sensitivity and specificity of GOG in detecting GIST were 91.80% and 88.50%, respectively, while the sensitivity and specificity of GOG in DOG1 and CD117 were 98.60% and 84.60%, respectively. The sensitivity of DOG1 (P> 0.05). There was no correlation between the expression of DOG1 and the prognosis of GIST (P> 0.05), but no correlation was found between the expression of DOG1 and the prognosis of GIST > 0.05). CONCLUSIONS: DOG1 is a sensitive and specific indicator of CD117 in GIST diagnosis, especially for the diagnosis of CD117-negative GIST. However, it can not be used as an indicator to evaluate the prognosis of GIST.
