目的 患者在低辐射、降低25%碘摄入量条件下进行扫描,原始数据应用基于原始数据的迭代技术(SAFIRE)进行重建,图像质量与常规方案扫描后的图像比较,旨在了解经SAFIRE重建的图像质量及其临床应用价值.方法 将58例临床疑似稳定性心绞痛患者随机分为两组,采用不同的管电压、碘用量和重建方法,A:100kvp、210mgI/kg对比剂、SAFIRE重建,B:120kvp、280mgI/kg对比剂、FBP重建.测量升主动脉开口(AR)、右冠状动脉(RCA)、左冠状动脉主干(LM)、左前降支(LAD)及左回旋支(LCX)的CT值、背景噪声(SD)并计算分析信号噪声比(SNR)、对比噪声比(CNR)值,比较两者的图像质量.结果 A组的碘摄入量明显低于B组(A:15.0±1.54g,B:19.7±3.0g,P0.05),两组图像质量的主观评分差异无统计学意义(P>0.05).结论 利用SAFIRE技术能够在低管电压、低碘摄入量的条件下获得满足诊断的图像.“,”Objective To investigate the image quality and radiation dose of CT coronary angiography (CCTA) using snogram affirmed iterative reconstruction ( SAFIRE) with lower kVp and 25%iodine intake reduction compared with standard dose CTA u-sing filtered back-projection (FBP).Methods 58 patients were randomly divided into two groups (29 for each):Group A, 100 kVp, 210 mgI/kg, and reconstructed with SAFIRE;group B,120 kVp, 280 mgI/kg and reconstructed with FBP .The CT attenua-tion values of aortic root (AR), left main coronary artery (LMA), right main coronary artery (RMA) , left circumflex artery (LCX) and background noise were measured.The CT dose index-volume (CTDI), Signal-to-noise ratio (SNR), and contrast-to-noise ratio ( CNR) were measured and statistically analysed .Image quality was evaluated by two senior radiologists using a quali-tative five-point scale.Results Compared with group B, group A significantly reduced the iodine intake (A:15.0 ±1.54g, B:19.7 ±3.0g, P 0.05).Conclusion Using SAFIRE under the condition of lower kvp and 25%iodine reduction could obtain satisfactory images meeting the diagnostic requirements .
