目的探讨体外受精-胚胎移植(IVF-ET)过程中基础卵泡刺激素(FSH)水平及年龄预测卵巢储备及IVF-ET结局的价值。方法对1999年5月至2006年3月在郑州大学第二附属医院生殖中心行IVF-ET助孕术的376个新鲜周期结局进行回顾性分析,比较不同FSH水平和不同年龄患者卵巢反应性和IVF-ET结局的差异。结果FSH<10 IU/L患者Gn用量明显低于10 IU/L≤FSH≤15 IU/L和FSH>15 IU/L组患者,而取卵数明显高于其他两组(P<0.05=。年龄>35岁患者的基础FSH值、Gn用量显著高于其他两组(P<0.05=,而取卵数、卵裂率、胚胎植入率、临床妊娠率和继续妊娠率均明显低于其他两组(P<0.05=。低龄高基础FSH(年龄<38岁且FSH≥10 IU/L=患者的胚胎植入率、临床妊娠率和继续妊娠率均显著高于高龄低基础FSH(年龄≥38岁且FSH<10 IU/L=患者(P<0.05=。不同FSH水平患者受精率、卵裂率、移植胚胎数(ET数)、胚胎植入率、临床妊娠率、流产率差异均无统计学意义(P>0.05)。结论基础FSH水平可以预测卵巢的储备功能和取卵数目,但不能预测IVF-ET的临床结局,而年龄对IVF-ET的临床结局有一定的预测价值。
Objective To investigate the value of basal follicle stimulating hormone (FSH) and age in prediction of ovarian reserve and outcome of IVF-ET in IVF-ET (IVF-ET). Methods A retrospective analysis of 376 fresh-cycle outcomes of IVF-ET assisted pregnancy in the Reproductive Center of the Second Affiliated Hospital of Zhengzhou University from May 1999 to March 2006 was conducted to compare the ovarian response of patients with different FSH levels and different ages Differences in IVF-ET outcome. Results The amount of Gn in patients with FSH <10 IU / L was significantly lower than that in patients with 10 IU / L≤FSH≤15 IU / L and FSH> 15 IU / L, and the number of oocytes retrieved was significantly higher than the other two groups (P <0.05). The basal FSH value and the amount of Gn in patients aged> 35 years old were significantly higher than those in the other two groups (P <0.05). The numbers of oocytes retrieved, cleavage rate, embryo implantation rate, clinical pregnancy rate and continuous pregnancy rate were significantly lower than those in the other two groups The rates of embryo implantation, clinical pregnancy and continued pregnancy were significantly higher in the younger age group with high basic FSH (age <38 years and FSH≥10 IU / L = 38 years old and FSH <10 IU / L = (P <0.05). There were no differences in fertilization rate, cleavage rate, ET number, embryo implantation rate, clinical pregnancy rate and abortion rate among patients with different FSH levels (P> 0.05) .Conclusion The basal level of FSH can predict the ovarian reserve function and the number of oocytes retrieved, but it can not predict the clinical outcome of IVF-ET. However, age has some predictive value for the clinical outcome of IVF-ET.
