目的:了解冠心病患者行经皮腔内冠状动脉成形术(PTCA)前后血清肌酸激酶-同工酶(CK-MB)、镁(Mg2+)的变化。方法:以16例冠状动脉造影(CAG)阴性者为阴性对照组,以10例CAG阳性但不宜行PTCA者为阳性对照组,观察24例行PTCA冠心病患者(PTCA组)手术前后血清CK-MB、Mg2+的动态变化。结果:PTCA组术后5、30、120min血清CK-MB较术前及同时点两对照组均有显著增加(P<0.01),Mg2+在术后5、30、120、360min较术前及同时点阴性对照组显著减少(P<0.05);阳性对照组Mg2+值在术后5min比术前及同时点阴性对照组显著减少(P<0.05及P<0.01)。另外,PTCA组术后30min血清CK-MB与Mg2+呈负相关(P<0.001)。结论:PTCA可引起一定程度的心肌损伤,但尚未造成心肌明显坏死;该研究可部分揭示PTCA后心肌缺血再灌注损伤的发生机制。“,”Objective:To understand the changes of levels of serum CK-MB andMg2+ of coronary heart disease(CHD) patients before and after PTCA.Method:Using 16 negative patients of coronary angiocardiography(CAG) serving as negative control group and 10 patients with positive CAG but unsuitable for PTCA as positive control group,we observed the dynamic changes of levels of serum CK-MB、Mg2+ of 24 CHD patients before and after PTCA(PTCA group).Result:In PTCA group,the levels of serum CK-MB were significantly higher at after PTCA at 5,30,120 min at than that before PTCA and those of the two control groups at the same time (P<0.01).In contrast,the levels of Mg2+ were significantly lower after PTCA at 5,30,120,360 min that than before PTCA and those in the negative group at same time (P<0.05).In the positive group,the level of Mg2+ was significantly lower after operation 5min than before operation and the negative group of the corresponding time(P<0.05 and P<0.01 respectively).In addition,there was negative relationship between CK-MB and Mg2+ at 30 min after PTCA (P<0.001).Conclusion:PTCA can cause myocardial damage to some degree but still does not lead to apparent necrosis of myocardium,this may partially explain the mechanism of myocardial ischemia/reperfusion injury after PTCA.
