目的探讨rSj26-Sj32-IgM-ELISA法用于急性日本血吸虫病的诊断价值。方法用纯化的rSj26-Sj32融合蛋白和日本血吸虫成虫抗原(SjAWA)建立ELISA方法,检测急性日本血吸虫病患者血清中的特异性IgM抗体,并以华支睾吸虫病、卫氏并殖吸虫病、棘球蚴病、乙型肝炎、肺结核病患者和健康人血清作对照,比较二者的敏感性、特异性和交叉反应性。结果 rSj26-Sj32-IgM-ELISA和SjAWA-IgM-ELISA检测急性日本血吸虫病患者血清特异性IgM抗体的敏感性分别为98.0%和96.0%,特异性均为97.7%;SjAWA-IgM-ELISA检测华支睾吸虫病和卫氏并殖吸虫病患者血清时存在不同程度的交叉反应,而rSj26-Sj32-IgM-ELISA则未见交叉反应。结论纯化的rSj26-Sj32融合蛋白对急性日本血吸虫病具有较高诊断价值,可用作急性日本血吸虫病的诊断抗原。
Objective To investigate the diagnostic value of rSj26-Sj32-IgM-ELISA in the diagnosis of acute Japanese schistosomiasis. Methods The purified rSj26-Sj32 fusion protein and the adult schistosoma japonicum antigen (SjAWA) were used to detect the specific IgM antibodies in the sera of patients with acute schistosomiasis japonica. ELISA was used to detect clonorchiasis, Paragonimus westermani, Echinococcosis, Hepatitis B, tuberculosis patients and healthy human serum as a control, comparing the sensitivity, specificity and cross-reactivity. Results The sensitivity and specificity of rSj26-Sj32-IgM-ELISA and SjAWA-IgM-ELISA for the detection of serum specific IgM antibodies in patients with acute schistosomiasis japonica were 98.0% and 96.0%, respectively, with a specificity of 97.7%. SjAWA-IgM- Clonorchiasis and Paragonimus westermani serum levels were different degrees of cross-reaction, and rSj26-Sj32-IgM-ELISA no cross-reaction. Conclusion The purified rSj26-Sj32 fusion protein has high diagnostic value for acute schistosomiasis japonica and can be used as a diagnostic antigen for acute schistosomiasis japonica.
