目的探讨经颅多普勒超声(TCD)在评价蛛网膜下腔出血(SAH)后脑血管痉挛(CVS)中的作用。方法回顾性分析连云港市第一人民医院2004年以来42例CVS患者的TCD检测结果,并与SAH发生时间、出血程度(F isher分级)和预后进行比较分析。结果病程<6 d组以轻度为主,7~15 d以中重度为主,出血少量组CVS以轻度为主,出血多量组以中重度为主(P<0.05)。SAH发生时间、程度对脑血管痉挛有明显影响,与SAH患者预后有关。结论经颅多普勒超声检测对原发性蛛网膜下腔出血后脑血管痉挛的程度评估和预后判断有重要意义。
Objective To investigate the role of transcranial Doppler ultrasound (TCD) in the evaluation of cerebral vasospasm (CVS) after subarachnoid hemorrhage (SAH). Methods The TCD results of 42 patients with CVS in First People’s Hospital of Lianyungang were retrospectively analyzed and compared with the time of occurrence of SAH, the degree of bleeding and the prognosis. Results The course of disease was mainly mild in <6 d group and moderate to severe in 7 ~ 15 d. The mild CVS was predominant in the small amount of hemorrhage and moderate to severe in the majority of hemorrhage (P <0.05). The time and extent of SAH have a significant effect on cerebral vasospasm, which is related to the prognosis of patients with SAH. Conclusion Transcranial Doppler ultrasonography is of great significance in the assessment of cerebrovascular spasm and prognosis of patients with primary subarachnoid hemorrhage.
