目的研究宫颈鳞癌癌变过程中HER-2、EGFR的表达及临床意义。方法利用组织芯片技术免疫组织化学和荧光原位杂交(FISH)技术探讨60例宫颈鳞癌组织,61例CIN以及21例正常宫颈组织中HER2及EGFR蛋白的表达及HER-2基因扩增的情况。结果从宫颈正常鳞状上皮、上皮内瘤变到癌的发展过程中HER2与EGFR的表达率呈现逐步上升的趋势(P<0.05)。HER2在三组中的表达率依次为0.00%、6.56%、31.67%。EGFR在三组中的表达率依次为0.00%、54.10%、71.67%。HER2的表达与宫颈鳞癌的分级、分期及淋巴结转移密切相关(P<0.05),EGFR的表达与宫颈鳞癌的分期与淋巴结转移密切相关(P<0.05),与宫颈鳞癌的分级无相关关系(P>0.05)。60例宫颈鳞癌标本,13例有基因扩增;30例CINⅢ级标本,1例有基因扩增;CINⅠ-Ⅱ级和正常组织标本均无扩增。结论 EGFR的表达与宫颈鳞癌的发生发展密切相关;HER2的蛋白表达与基因扩增可能参与调控了宫颈鳞癌的发生发展过程。
Objective To study the expression and clinical significance of HER-2 and EGFR in cervical carcinogenesis of cervical squamous cell carcinoma. Methods The expression of HER2 and EGFR protein and the amplification of HER-2 gene in 60 cases of cervical squamous cell carcinoma, 61 cases of CIN and 21 cases of normal cervical tissue were studied by tissue microarray immunohistochemistry and fluorescence in situ hybridization (FISH) . Results The expression rates of HER2 and EGFR increased gradually from normal squamous cell carcinoma and intraepithelial neoplasia to carcinogenesis (P <0.05). HER2 expression rate in the three groups were 0.00%, 6.56%, 31.67%. The expression rates of EGFR in three groups were 0.00%, 54.10% and 71.67%, respectively. The expression of HER2 was closely related to the grade, stage and lymph node metastasis (P <0.05). The expression of EGFR was closely related to the stage and lymph node metastasis (P <0.05), but not to the grade of cervical squamous cell carcinoma Relationship (P> 0.05). 60 cases of cervical squamous cell carcinoma specimens, 13 cases of gene amplification; 30 cases of CIN Ⅲ grade specimens, 1 case of gene amplification; CIN Ⅰ-Ⅱ grade and normal tissue specimens were no amplification. Conclusion The expression of EGFR is closely related to the occurrence and development of cervical squamous cell carcinoma. The protein expression and gene amplification of HER2 may be involved in the regulation of cervical squamous cell carcinoma.
