目的:探讨经尿道绿激光膀胱肿瘤汽化术的手术方法、近期疗效及与其它文献方法的比较。方法:采用骶管或硬膜外腔阻滞麻醉,应用美国Laserscope公司生产的非接触式绿激光治疗系统,以30～80W功率对膀胱肿瘤进行汽化。结果:本组汽化时间10～70min,耗能12.23～86.70kJ,出血0～10 ml,术后113例未留置尿管,余67例留置尿管24～96h,膀胱冲洗0～72h,术后住院3～9d,术中均未输血,均未转为开放手术。结论:绿激光膀胱肿瘤汽化术手术安全、操作简单、手术时间短、出血少、痛苦小、留置尿管时间短、康复快,无死角、盲区,可重复性强,是一种理想的微创手术方法。
Objective: To investigate the surgical methods, short-term curative effect and comparison with other literatures of transurethral green-laser bladder tumor vaporization. Methods: The caudal or epidural block anesthesia was applied to the non-contact green laser treatment system manufactured by Laserscope in the United States to vaporize the bladder tumor with power of 30-80W. Results: The time of vaporization in this group was 10-70min, the energy consumption was 12.23-86.70kJ, the volume of hemorrhage was 0-10ml. In the postoperative period, 113 cases had no indwelling catheter, the other 67 cases had indwelling catheter for 24-96h, the bladder was irrigated for 0-72h, Hospitalized 3 ~ 9d, intraoperative blood transfusion, were not converted to open surgery. Conclusion: The green laser bladder tumor is safe and easy to operate with simple operation, short operation time, less bleeding, less pain, shorter indwelling catheter, faster recovery, no dead angle, blind spot and strong repeatability. It is an ideal minimally invasive Surgical methods.
