目的:通过对大肠腺瘤早期癌变核形态的量化分析,探讨其变化规律及应用于临床诊断的可能性。方法:应用图像分析的方法,对139例大肠病变标本进行核形态定量学研究。结果:腺瘤不典型增生至早期癌变,其细胞核形态的各项参数值均随恶性程度的增加而增大,组间差别有显著性(P<0.01)。通过参数逐类判别建立的函数式,其四代矩阵分析与原诊断的符合率为71%.结论:核形态图像量化分析对大肠腺瘤早期癌变具一定的鉴别能力,建立的函数判别式显示出一定的判别功能。
OBJECTIVE: To quantitatively analyze the nuclear morphology of the early stage of large intestine adenoma, and to explore its changing rules and the possibility of applying it to clinical diagnosis. Methods: Image analysis was used to study nuclear morphometry in 139 specimens of colorectal lesions. Results: The adenoma showed atypical hyperplasia to early carcinogenesis. The morphological parameters of the karyotype increased with the increase of malignancy. The difference between the two groups was significant (P<0.01). The functional formulas established by the parameters were discriminated on a class-by-class basis. The coincidence rate of the four-generation matrix analysis and the original diagnosis was 71%. Conclusion: Quantitative analysis of nuclear morphology has a certain ability to discriminate the early stage of colorectal adenoma. The function discriminant equation established shows a certain discriminant function.
