本文报道用99m_Tc-MDP对36例股骨颈骨折或骨折愈合后发生股骨头塌陷者进行了放射性核素骨显像(RNBI)随访检查,其中11例结合单光子发射型计算机断层(SPECT)检查。结果表明:(1)根据骨折后骨显像的动态变化,可较准确地了解股骨头坏死的修复过程,预测后期股骨头塌陷。(2)股骨头塌陷前期及塌陷后均呈现高代谢反应,17例股骨头塌陷者的平均患头/健头比为2.22,表明迟发性高代谢反应与塌陷有密切关系,因此,局部核素摄取量增加不能作为治疗有效的依据。(3)SPECT检查可去除平面骨显像的放射性重叠,对分析股骨头各断层的坏死和修复状况,有重要价值。
This paper reports the follow-up examination of radionuclide bone imaging (RNBI) in 36 cases of femoral neck fracture or fracture healing after fracture healing with 99m_Tc-MDP. Eleven cases were examined by single photon emission computed tomography (SPECT). The results showed that: (1) According to the dynamic changes of bone imaging after fracture, the repair process of femoral head necrosis can be more accurately understood and the collapse of the femoral head can be predicted. (2) Both the early femoral head collapse and the post-collapse femoral head showed a high metabolic response, and the average head-head-to-head ratio was 2.22 in 17 femoral head collapsed patients, indicating that late-onset hypermetabolism is closely related to collapse. Therefore, Increased local intake of radionuclide can not be used as a basis for effective treatment. (3) SPECT examination can remove the radioactive overlap of planar bone imaging, the analysis of the various sections of the femoral head necrosis and repair of great value.
