目的 观察缺氧缺血性脑病 (HIE)患儿电解质、血糖、血气分析的变化 ,探讨其变化与疾病程度的关系。方法 对 85例于生后 2天内收入院的HIE患儿 ,按疾病程度分做轻度组 (4 6例 )和中重度组 (3 9例 ) ,在开始治疗前 ,采静脉血 3ml,查电解质K+ 、Na+ 、Cl- 、Ca2 + 及血糖 ,采动脉血 1ml行血气分析 ,并选择生后 2天内无围产期窒息史 ,母无糖尿病史的 40例足月儿做对照。结果 HIE患儿电解质改变有统计学意义的是血钠变化 ,轻度HIE组血钠 13 5 2± 2 2 4mmol/L ,和对照组相比有差异 (P <0 0 5 ) ,中重度组血钠 13 0 45± 3 15mmol/L ,显著低于轻度组 (P <0 0 1) ;血糖轻度组 5 15± 0 84mmol/L ,中重度组 4 49± 0 42mmol/L ,表现为随疾病程度加重 ,血糖值有显著下降 (P <0 0 1)。结论 HIE患儿病情越重 ,越易出现电解质变化 (低血钠 )、血糖下降 ,临床上应严密监测 ,及早用药治疗
Objective To observe the changes of electrolytes, blood glucose and blood gas in children with hypoxic-ischemic encephalopathy (HIE) and to explore the relationship between changes and disease severity. Methods Eighty-five HIE infants admitted to hospital within 2 days after birth were divided into mild group (46 cases) and moderate-severe group (39 cases) according to the degree of disease. Before the treatment, 3ml venous blood Electrolytes K +, Na +, Cl-, Ca2 + and blood glucose, blood sampling 1ml arterial blood gas analysis, and select 2 days after birth without perinatal asphyxia history, no history of diabetes in 40 cases of full-term children as controls. Results Electrolytes in children with HIE had statistical significance in the change of serum sodium. Hypothyroidism was 13 52 ± 2 24 mmol / L in HIE group, which was significantly lower than that in control group (P <0.05) Serum sodium was 13 0 45 ± 3 15mmol / L, significantly lower than mild group (P <0.01); mild blood glucose group 5 15 ± 0 84mmol / L, moderate and severe group 4 49 ± 0 42mmol / L, With the severity of the disease, blood glucose levels decreased significantly (P <0.01). Conclusion The more serious the disease in children with HIE, the more prone to electrolyte changes (hyponatremia), blood glucose decline, the clinical should be closely monitored, and early medication
