目的 研究幽门螺杆菌 (Helicobacterpylori,Hp)感染胃黏膜病变的多基因表达和细胞生物学行为。方法 32 7例患者经胃镜及手术的胃黏膜病变标本 ,应用免疫组化染色法 ,检测p5 3、p16、bcl 2和环氧合酶同工酶 2 (COX 2 )蛋白的表达。Hp感染由快速尿素酶试验结合组织学检查 /14 C尿素呼气试验而确立。 6 6例伴肠化和 /或异型增生患者的标本作细胞图像分析。结果 p5 3在胃癌中的阳性表达率显著高于异型增生组 (P <0 .0 5 )。在Hp阳性慢性萎缩性胃炎、肠化或异型增生组 ,p16表达均显著低于Hp阴性组 (P <0 .0 5 ,P <0 .0 1) ,在Hp阳性肠化或异型增生组 ,bcl 2或COX 2的表达显著高于Hp阴性组 (P <0 .0 5 )。细胞图像分析表明 :Hp阳性肠化和 /或异型增生组 ,大部分参数的均数都显著高于Hp阴性组 (P <0 .0 5 ,P <0 .0 1)。 结论 p16、bcl 2和COX 2基因在胃癌早期发生发展过程中起重要作用 ,并与Hp感染有一定关系。在胃肿瘤发生中 ,p5 3基因变化为较晚期事件 ,与Hp感染无明显关系。Hp感染的胃癌前病变具有更多的恶性肿瘤生物学行为
Objective To investigate the multi-gene expression and cell biological behavior of gastric mucosal lesions in patients with Helicobacter pylori (Hp) infection. Methods Thirty-seven gastric biopsy specimens of gastric mucosa were collected. The expressions of p53, p16, bcl2 and cyclooxygenase-2 (COX-2) were detected by immunohistochemical staining. Hp infection was confirmed by a rapid urease test in combination with a histological examination / 14 C urea breath test. Sixty-six specimens of patients with intestinal metaplasia and / or dysplasia were analyzed for cell images. Results The positive rate of p5 3 in gastric cancer was significantly higher than that in dysplasia (P <0.05). The expression of p16 in Hp positive chronic atrophic gastritis, intestinal metaplasia or dysplasia group was significantly lower than that in Hp negative group (P <0.05, P <0.01). In Hp positive intestinal metaplasia or dysplasia group, The expression of bcl 2 or COX 2 was significantly higher than that of Hp negative group (P <0.05). Cell image analysis showed that the average number of most parameters in Hp positive intestinal metaplasia and / or dysplasia group was significantly higher than that in Hp negative group (P <0.05, P <0.01). Conclusion The p16, bcl 2 and COX 2 genes play an important role in the early development of gastric cancer and have some relationship with Hp infection. In gastric carcinogenesis, p5 3 gene changes to a more advanced event, and no significant relationship with Hp infection. Hp-infected gastric precancerous lesions have more malignant biological behavior
