本文对17例(28例次)慢性阻塞性肺病所致呼衰采用高频喷射通气(HFJV)的临床资料进行了分析,重点比较通气频率对血气改变的影响;资料表明:HFJV对慢阻肺所致的Ⅰ、Ⅱ型呼衰能有效的改善低氧血症,但在Ⅱ型呼衰随着PaO_2的改善PaCO_2亦随之上升而导致CO_2潴留;故对Ⅱ型呼衰应慎用。应用HFJV时应注意调整有关参数,初步认为驱动压0.8~1.0kg/cm~2,吸呼比为1:2~3,通气频率为30~60次/min是HFJV治疗慢阻肺呼衰较理想的通气参数。
In this paper, 17 cases (28 cases) of chronic obstructive pulmonary disease caused by respiratory failure using high frequency jet ventilation (HFJV) clinical data were analyzed, focusing on the comparison of ventilation frequency on blood gas changes; data show that: HFJV on chronic obstructive pulmonary The type Ⅰ and type Ⅱ respiratory failure can effectively improve the hypoxemia, but the type Ⅱ respiratory failure with the PaO_2 improved PaCO_2 also increased with the resulting CO_2 retention; type Ⅱ respiratory failure should be used with caution.
