目的研究T4K矫治器对生长发育期安氏Ⅱ~1错儿童颅颌面功能及结构的影响,为临床应用提供理论及实践支持。方法按照纳入标准选择2014年6月至2015年6月到深圳市罗湖区人民医院口腔医学中心就诊的20例安氏Ⅱ~1错儿童患者作为试验组,另选择同期到本院门诊就诊咨询,符合纳入标准但拒绝治疗的20例安氏Ⅱ~1错儿童患者作为对照组。试验组患者采用T4K矫治器治疗,对照组患者未做任何治疗仅观察。在同等治疗或观察期后比较两组患者颅颌面硬组织的变化。结果试验组患者经T4K矫治器治疗后,SNB(蝶鞍点-鼻根点-下牙槽座点角)、L1-MP(下中切牙与下颌平面相交之后上角)、S-Go(后面高)、N-Me(前面高)比对照组患者有显著增大;而ANB(上牙槽座点-鼻根点-下牙槽座点角)、U1-SN(上中切牙长轴与SN平面相交的下内角)、overbite(覆)、overjet(覆盖)比对照组患者有显著减小,差异有统计学意义(P<0.05)。结论 T4K矫治器用于治疗儿童安氏Ⅱ~1错患者可获得良好的侧貌,但有一部分矫治效果是来自于患者颅颌面的自然生长。所以对儿童安氏Ⅱ~1错患者进行T4K矫治器早期矫治,可充分利用其生长发育的潜力,达到事半功倍的效果。
Objective To study the effects of T4K appliance on the craniomaxillofacial function and structure of children with Angle Ⅱ ~ 1 malocclusion during growth and development, and to provide theoretical and practical support for clinical application. Methods According to the inclusion criteria, 20 cases of children with Angle Ⅱ ~ 1 malocclusion admitted to the Stomatology Center of People’s Hospital of Luohu District, Shenzhen from June 2014 to June 2015 were selected as the experimental group. In the meantime, 20 children with malocclusion Ⅱ ~ 1 who met the inclusion criteria but refused treatment were taken as the control group. The patients in the test group were treated with T4K appliance and the patients in the control group were observed only without any treatment. Craniomaxillofacial changes of the two groups were compared after the same treatment or observation period. Results After treatment with T4K appliance in the test group, SNB (angle between the sellae - nasal root - inferior alveolar), L1-MP (upper angle after the inferior incisors intersect the mandibular plane), S-Go High), N-Me (anterior height) than the control group patients increased significantly; while ANB (upper alveolar-nasal point - the lower alveolar niche), U1-SN And the lower plane of intersection with SN plane), overbite and overjet were significantly reduced compared with the control group (P <0.05). Conclusions T4K appliance can be used to treat patients with Angle Ⅱ ~ 1 malformations, but some of the orthodontic effects come from the craniofacial growth. Therefore, children with Angle Ⅱ ~ 1 wrong patients with T4K appliance early treatment, can take full advantage of its potential for growth and development, to achieve a multiplier effect.
