目的探讨64排螺旋CT血管成像对Adamkiewicz动脉显示的最佳参数。方法将84例受试者按检查序号分为4组,A组24例,对比剂流速为3ml/s,开始注射对比剂后25s,CT机自动开始扫描。B组20例,对比剂流速为4ml/s,开始注射对比剂后25s,CT机自动开始扫描。C组20例,对比剂流速为5ml/s,开始注射对比剂后25s,CT机自动开始扫描。D组20例,流速4ml/s,应用自动阈值触发技术,当降主动脉内感兴趣区相对衰减值达到230HU后,再延迟3s,CT机自动触发进行扫描。将原始图像进行容积显示(volume rendering,VR)、多平面重建(multiplanar reformation,MPR)、曲面重建(curved planar reformation,CPR),观察和分析Adamkiewicz动脉的显示情况,对不同参数下的Adamkiewicz动脉显示效果进行比较。结果总显示率为85.71%,对比剂流速4ml/s的B组及对比剂流速5ml/s的C组显示率高于3ml/s的A组,应用自动阈值触发技术的D组Adamkiewicz动脉显影效果优于A组、B组。结论将对比剂流速控制在4ml/s以上,以T9水平降主动脉感兴区相对衰减值达到230HU作为阈值自动触发64排螺旋CT进行血管成像,可以成功地显示Adamkiewicz动脉。
Objective To explore the optimal parameters of Adamkiewicz artery display by 64-slice spiral CT angiography. Methods Eighty-four subjects were divided into 4 groups according to examination sequence, 24 cases in group A, the contrast agent flow rate was 3ml / s, and 25 seconds after the start of injection of contrast agent, the CT machine began to scan automatically. B group of 20 cases, the contrast agent flow rate of 4ml / s, 25s after the start of contrast agent injection, CT machine automatically start scanning. C group 20 cases, contrast agent flow rate of 5ml / s, began injecting contrast agent after 25s, CT machine automatically start scanning. In group D, the flow rate was 4ml / s and flow rate was 4ml / s. When the relative attenuation value of the region of interest in the descending aorta reached 230HU, then delayed by 3s, the CT machine automatically triggered the scanning. The volume rendering (VR), multiplanar reformation (MPR) and curved planar reformation (CPR) of the original image were performed to observe and analyze the Adamkiewicz artery. The results showed that the total display rate was 85.71%, the contrast agent flow rate of 4ml / s in group B and the contrast agent flow rate of 5ml / s in group C was higher than 3ml / s in group A, the application of automatic threshold-triggered group D Adamkiewicz arterial imaging Better than A group, B group. Conclusion The contrast agent flow rate is controlled above 4ml / s, and the relative attenuation value of T9 level descending aortic afferent area reaches 230HU as the threshold to automatically trigger the 64-slice spiral CT angiography, which can successfully display the Adamkiewicz artery.
