经腹全子宫切除是妇科最常见的手术,单极电刀在该术式中已有应用。最近,外科器械中又出现了一个新成员一双极电剪,并在腹腔镜手术中得到广泛应用。然而,有关双极电剪在妇科手术过程中的应用尚未见报道。双极电剪具有切割及凝固组织的双重功能,可减少术中结扎次数,减少出血量。本文首次报道了双极电剪在经腹全子宫切除术中的应用,并与常规子宫切除术相比较,以探讨其优缺点。 子宫肌瘤患者50例,随机分为2组:采用常规方法经腹全子宫切除为常规组,手术过程中应用双极电剪为双极电剪组。每组25例,两组间年龄、体重、子宫重量等均无差异。双极电剪易于使用,其形状与普通的外科剪相同,有一绝缘尼龙手柄,剪的两叶由一薄层陶瓷隔绝,因而具有双极性。记录手术时间(切皮至缝皮时间)、结扎次数、切除的子宫重
Transabdominal hysterectomy is the most common gynecological surgery, monopolar electrotome has been used in this technique. Recently, a new member of bipolar scissors appeared in surgical instruments and has been widely used in laparoscopic surgery. However, the application of bipolar shear in gynecological surgery has not been reported yet. Bipolar shear has the dual function of cutting and coagulating tissue, which can reduce the number of intraoperative ligation and reduce the amount of bleeding. This article for the first time reported the bipolar shear in the application of abdominal hysterectomy, and compared with conventional hysterectomy to explore its advantages and disadvantages. 50 patients with uterine fibroids were randomly divided into two groups: conventional abdominal radical hysterectomy as the conventional group, the application of bipolar electrical shear bipolar shear group during surgery.
