目的探讨血清半胱氨酸蛋白酶抑制剂C(Cys-C)和腺苷脱氨酶(ADA)联合检测在慢性肝病中的临床意义。方法收集中山大学孙逸仙纪念医院慢性肝病患者血清标本105例和健康体检者血清标本35例,根据中华医学会肝病学分会修订的《肝病诊治指南》,分为慢性肝炎组35例、肝硬化组35例和肝癌组35例。采用贝克曼AU5800全自动生化仪测定肝病组和对照组血清Cys-C和ADA水平。结果各肝病组血清Cys-C、ADA均高于对照组,肝硬化组、肝癌组均高于慢性肝炎组,肝硬化组高于肝癌组,差异均有统计学意义(P<0.05)。联合检测血清Cys-C和ADA,各肝病组异常率较单独检测血清Cys-C或ADA均明显增高。Pearson直线相关分析显示,血清Cys-C水平与ADA呈正相关(r=0.505,P<0.01)。结论慢性肝病患者血清Cys-C水平明显增高,可作为肝功能评价指标。ADA在对肝硬化的诊断方面优于Cys-C,且对各肝病组阳性率高于Cys-C,联合检测Cys-C和ADA对监测慢性肝病病情进展具有重要的参考价值。
Objective To investigate the clinical significance of combined detection of serum cystatin C (Cys-C) and adenosine deaminase (ADA) in chronic liver disease. Methods 105 cases of serum samples of chronic liver disease patients from Sun Yat-sen Memorial Hospital of Sun Yat-sen University and 35 cases of serum samples from healthy subjects were collected. According to the Guide to Diagnosis and Treatment of Liver Diseases revised by Chinese Medical Association Liver Diseases Branch, 35 cases were divided into chronic hepatitis group, cirrhosis group 35 Cases and liver cancer group of 35 cases. The serum levels of Cys-C and ADA in liver disease group and control group were determined by Beckman AU5800 automatic biochemical analyzer. Results The serum levels of Cys-C and ADA in each liver disease group were significantly higher than those in control group. The levels of serum Cys-C and ADA in cirrhosis and hepatocellular carcinoma were higher than those in chronic hepatitis group and cirrhosis group (P <0.05). Joint detection of serum Cys-C and ADA, liver disease abnormalities than the single test serum Cys-C or ADA were significantly higher. Pearson linear correlation analysis showed that serum Cys-C level was positively correlated with ADA (r = 0.505, P <0.01). Conclusion Serum levels of Cys-C in patients with chronic liver disease are significantly increased, which can be used as an index to evaluate liver function. ADA is superior to Cys-C in the diagnosis of cirrhosis, and its positive rate is higher than that of Cys-C in each liver disease group. Combined detection of Cys-C and ADA has important reference value for monitoring the progression of chronic liver disease.
