59岁女性患者,患类风湿性关节炎14年,从未服金制剂、青霉胺等药物,尿液检查发现中至大量蛋白尿及少量镜下血尿,肾活检组织学改变符合肾小球膜性病变,同时伴新月体和袢坏死;实验室检查证实抗中性粒细胞胞质抗体阳性。结合临床考虑类风湿性关节炎肾损害。
59-year-old female patient suffering from rheumatoid arthritis for 14 years, never taking gold preparations, penicillamine and other drugs, urine examination found that a large amount of proteinuria and a small amount of microscopic hematuria, renal biopsy histological changes consistent with glomerular Membranous lesions, accompanied by crescent and tarsal necrosis; laboratory tests confirmed anti-neutrophil cytoplasmic antibody positive. Combined with clinical considerations of renal damage in rheumatoid arthritis.
