目的探讨心外膜脂肪与2型糖尿病患者早期心功能改变相关性。方法对2013年10月到2014年9月北京安贞医院内分泌科所有研究对象相关指标进行统计分析,收集研究对象一般临床资料、测定生化指标、评价心功能,测量心外膜脂肪体积。结果 (1)2型糖尿病组腰围、体重指数、空腹血糖、糖化血红蛋白、三酰甘油、心外膜脂肪体积明显高于对照组(P<0.05),2型糖尿病组左室壁收缩期应变率(SRs)、舒张早期应变率(SRe)减低(P<0.05)。(2)多元线性回归分析显示,心外膜脂肪体积与左室壁后间隔基底段收缩期应变率(SRs)、下壁中段舒张早期应变率(SRe)密切相关。结论心外膜脂肪体积与2型糖尿病早期心功能改变密切相关,提示心外膜脂肪可能是2型糖尿病心功能改变新的危险因素及潜在的治疗靶点。
Objective To investigate the correlation between epicardial fat and early cardiac function in type 2 diabetic patients. Methods From October 2013 to September 2014, the related indicators of all subjects in the Department of Endocrinology of Beijing Anzhen Hospital were collected. The clinical data of the subjects were collected, the biochemical indexes were measured, the cardiac function was evaluated, and the epicardial fat volume was measured. Results (1) The waist circumference, body mass index, fasting blood glucose, glycosylated hemoglobin, triglyceride and epicardial fat volume in type 2 diabetes mellitus were significantly higher than those in control group (P <0.05). The incidence of left ventricular systolic strain (SRs) and early diastolic strain (SRe) decreased (P <0.05). (2) Multivariate linear regression analysis showed that the epicardial fat volume was closely related to the systolic strain rate (SRs) and the early diastolic strain rate (SRe) in the basal segment of the posterior wall of the left ventricular wall. Conclusions The epicardial fat volume is closely related to the early changes of cardiac function in patients with type 2 diabetes mellitus, suggesting that epicardial fat may be a new risk factor for cardiac function changes and a potential therapeutic target for type 2 diabetes mellitus.
