男患,3个月。2天前发热、咳嗽、气促,在乡卫生院按“感冒”治疗无效,6小时前因烦躁不安,腹胀、呕吐而急诊入院.查体:T39℃ R60次/分,BP13.3/7.7Pka,体重5kg.呻吟、烦躁、气促,前囟稍隆起,鼻翼扇动,口唇紫绀。咽部充血。两肺散在细湿罗音。心率200次/分,第一心音低,无杂音。腹胀,肝肋下平脐,剑下4cm,肠鸣音减弱。化验:血红蛋白140g/L,白细胞11.2×10~9/L.中性39％,淋巴60％,嗜酸性1％;血小板120×10~9/L,尿常规检查蛋白(++),白细胞0—1。血
