应用免疫组化S-P法,研究47例原发性肺癌手术标本中nm23-H1,PCNA,CD15的表达及其与区域性淋巴结转移的关系。结果显示:在原发性肺癌中nm23-H1,PC-NA,CD15的阳性表达分别为51.06%,65.96%,51.06%。其中nm23-H1在鳞癌中阳性表达43.3%(13/30),腺癌64.71%(11/17),腺瘤表达较鳞癌高。在鳞癌中无肺门及纵隔淋巴结转移者阳性62.50%(10/16),有转移者阳性21.41%(3/14)(P<0.01),显示nm23-H1表达与鳞癌的肺门及纵隔淋巴结转移呈负相关。在PCNA表达中,有区域淋巴结转移的阳性表达为82.6%(19/23),无淋巴结转移为54.16(13/24)(P<0.01),低分化癌阳性表达88.8%(8/9),明显高于高分化癌43.75%(7/16),(P<0.01)。在CD15表达中,有淋巴结转移组中阳性表达为78.26%(18/23),无区域性淋巴结转移组中阳性表达25%(P<0.01)。而nm23-H1,PCNA,CD15表达之间无显著相关性。本研究提示nm23-H1,PCNA,CD15的表达对肺癌预后评估有重要意义。
Immunohistochemical S-P method was used to study the expression of nm23-H1, PCNA and CD15 in 47 specimens of primary lung cancer and its relationship with regional lymph node metastasis. The results showed that the positive expression of nm23-H1, PC-NA and CD15 in primary lung cancer were 51.06%, 65.96% and 51.06%, respectively. Among them, nm23-H1 was positively expressed in squamous cell carcinoma by 43.3% (13/30) and adenocarcinoma by 64.71% (11/17). The expression of adenoma was higher than that of squamous cell carcinoma. No hilar and mediastinal lymph node metastases were positive in 62.50% (10/16) of the squamous cell carcinomas and 21.41% (3/14) were positive in the metastasis (P<0.01), indicating the expression of nm23-H1 The hilar and septal lymph node metastases of squamous cell carcinoma were negatively correlated. In the expression of PCNA, the positive expression of regional lymph node metastasis was 82.6% (19/23), no lymph node metastasis was 54.16 (13/24) (P<0.01), and poorly differentiated cancer was positively expressed. 8% (8/9) was significantly higher than that of well-differentiated carcinoma 43.75% (7/16) (P<0.01). In CD15 expression, the positive expression was 78.26% (18/23) in the lymph node metastasis group and 25% in the no regional lymph node metastasis group (P<0.01). There was no significant correlation between the expression of nm23-H1, PCNA and CD15. This study suggests that the expression of nm23-H1, PCNA and CD15 is of great significance in the evaluation of prognosis of lung cancer.
