目的探讨电视胸腔镜术中动脉血气分析的特点。方法对22例胸腔镜手术患者行进胸前双肺通气、术中单肺通气及术后双肺通气三种状态下血气检测。结果术中单肺通气时动脉血氧分压(PaO2)明显降低,血氧饱和度(SaO2)同时降低,动脉血二氧化碳分压(PaCO2)无明显变化。结论手术过程中,术者操作与麻醉监控之间的配合、及时调整机械通气的方式是预防和纠正术中低氧血症的关键。
Objective To investigate the characteristics of arterial blood gas analysis during video-assisted thoracoscopic surgery. Methods Twenty-two thoracoscopic surgery patients underwent preoperative chest lung ventilation, intraoperative single lung ventilation, and postoperative double lung ventilation. Results During the single-lung ventilation, the arterial blood oxygen pressure (PaO2) decreased significantly, the blood oxygen saturation (SaO2) decreased, and the arterial blood carbon dioxide pressure (PaCO2) did not change significantly. Conclusion During the operation, the cooperation between the operator’s operation and the monitoring of the anesthesia and the timely adjustment of the mechanical ventilation are the key to preventing and correcting hypoxemia during the operation.
